Once you have been selected to a BSA Celtic team, please download and review the forms below. You will need to complete the forms and bring them to one of the club's registration days along with your payment.- Player Registration Checklist

- Medical Release Form (Must be notarized! Sign only in the presence of a Notary Public.) pdf
